Episodes of Care: When Do They Begin and When Do They End?

Thursday, June 2, 2016
2:00 pm - 3:30 pm Eastern

Speaker: Cathie Biga, President & CEO, Cardiovascular Management of Illinois

With the anticipated release of the cardiac "bundle" on May 20th, this webinar will focus on understanding episodes of care. The ACA mandated the "creation" of episodes, but many don't know where to begin (in fact, they can't figure out how to find and download them). Our Supplemental QRUR is a treasure trove of data and information that can help. We'll review the reports on the 64 episodes contained in our s-QRUR, as well as review the 40 episodes presented in the cost element of the proposed MIPS rule, so we can better analyze our data and define appropriate action plans. In addition, if the cardiac episode is released, we'll review the implications of that. If it is delayed, we'll review the outline that has already been distributed. And, as always, we will leave plenty of time for your questions on this.

Syncing Provider Compensation with a Value Economy

Thursday, June 16, 2016
1:00 pm - 2:00 pm Eastern

Speakers: Joel Sauer, VP, MedAxiom Consulting

The new value economy is challenging our historical incentives and creating an imbalance between organizations, in terms of rewards. Whenever such disparities exist, achieving top performance is challenging. Medicare and other payers are now rewarding top performers--and penalizing the bottom tier--so the stakes are higher than ever. Additionally, it's no longer simply about personal outcomes, or those of the organization; rather, the entire care continuum that's being judged. Co-management programs can be a very useful tool for creating economic alignment across provider entities. Whether with a single group or multiple parties, a carefully- crafted program can eliminate, or at least mitigate, financial disincentives and replace them with appropriate rewards. In this way, regardless of who ultimately is receiving the payer reward, the parties most responsible for driving patient outcomes will be in-sync with driving positive results.

Webinar Takeaways:

  • Learn the new elements of the value economy that can create economic imbalance between providers.
  • Understand the basic elements of co-management necessary for success.
  • Explore several different real-life models that have been used successfully to align providers.

APP Productivity and Utilization: Understanding Barriers and Opportunities (Part 2)

Tuesday, July 12, 2016
1:00 pm - 2:00 pm Eastern

Speakers: Ginger Biesbrock, VP - Consulting, MedAxiom Inc. and Joel Sauer, VP, MedAxiom Consulting.

After our review of productivity, this webinar will explore current compensation models--both physician and APP models--and how they can either be conflicting or synergistic to the encouragement of the team-based-care model. Our experience has found that limited utilization of APPs in a program can start with a compensation model that creates a conflict of interest--and a concern for competition among the providers.

This webinar will provide:

  • Insights into how compensation models can either provide support for team-based care models or disincentives for their use.
  • Several examples of successful compensation models that will provide support for team-based care and APP productivity in a positive environment.

APP Productivity and Utilization: Understanding Barriers and Opportunities (Part 3)

Tuesday, August 16, 2016
1:00 pm - 2:00 pm Eastern

Speakers: Ginger Biesbrock, VP - Consulting, MedAxiom Inc. and Suzette Jaskie, CEO, MedAxiom Consulting.

After our review of productivity and compensation, this webinar will provide the concept of 'systems of care' and the requirements for the way we deliver care, moving from fee- for-service to value. Even more so, developing a program that truly manages our patients' needs and has the ability to meet them where they are. We will explore the use of APPs in these systems, as it relates to disease diagnosis and management, heart failure, atrial fibrillation, structural heart and other specialty areas.

This webinar will allow you to:

  • View your program with the 'systems of care' framework in mind, to better understand what may be missing.
  • Give examples of systems of care and the utilization of APPs, as part of the team- based care model, in effective, efficient and cost-effective ways.
