Cardiac rehabilitation is a proven intervention to improve patient outcomes, reduce readmissions and engage patients post discharge. Despite its well-documented benefits, less than 20% of patients participate in rehab programs. As health care rapidly adopts value-based reimbursement models, novel approaches are needed to increase cardiac rehab participation. This webinar highlights evidence for the importance of cardiac rehab and explores proven strategies, tools and new technologies for expanding the impact of cardiac rehab to increase patient engagement and adherence, including home-based and hybrid models. It will also cover the business case for cardiac rehab within the 90-day cardiac bundle.

A recently released health policy statement by the American College of Cardiology (ACC), in conjunction with the American Society of Echocardiography, American Society of Nuclear Cardiology, Heart Rhythm Society, and Society for Cardiovascular Angiography and Interventions, documents the College’s official position on the need for a standards-based approach to achieve interoperability of health information and to engage clinicians, manufacturers, and regulators in the process.
This panel discussion, including one of the policy statement authors, will explore the importance of and suggested path to interoperability: “Meaningful interoperability of data is crucial for optimal patient care as well as the many associated activities necessary to support a robust and transparent healthcare delivery system.
