The road to success when developing a chronic heart failure clinic can be a bumpy one. This presentation sheds light on Sanger Heart & Vascular Institute’s journey from failure to success in its approach to preventing hospital readmissions for CHF. Learn how Sanger designed and implemented innovative initiatives including: a transitional care clinic called Heart Success to address 30 day all cause readmission concern; a readmission reduction program; inpatient nurse navigator and multidisciplinary team approach to managing the patients; a virtual heart success program for addressing patient non-capture at smaller regional facilities; an integrated practice unit (IPU) designed to intensively manage chronic complex medical patients during the first 30 days post hospitalization to improve care for patients and prevent unnecessary ED visits and hospital readmissions (including a community paramedicine program) and; heart failure diagnostics to manage patients remotely and prevent heart failure index admissions and readmissions.
