Cardiac rehabilitation is a proven intervention to improve patient outcomes, reduce readmissions and engage patients post discharge. Despite its well-documented benefits, less than 20% of patients participate in rehab programs. As health care rapidly adopts value-based reimbursement models, novel approaches are needed to increase cardiac rehab participation. This webinar highlights evidence for the importance of cardiac rehab and explores proven strategies, tools and new technologies for expanding the impact of cardiac rehab to increase patient engagement and adherence, including home-based and hybrid models.
