Remote patient monitoring (RPM) and chronic care management (CCM) are Medicare care management programs that enable proactive care between office visits, improve access to personalized care, and reduce utilization. This session will dissect the need for electrophysiology (EP) program optimization to align with the recently published atrial fibrillation guidelines and evolving indications for EP interventions. This session will provide a framework for systematic cardiac triage, transfer and outpatient care coordination. Speakers will explore the benefits of early cardiac triage of patients prior to admission to the hospital or interhospital transfer in the context of limitations in bed capacity, transfer and inpatient access.

This session will illustrate an alternative care delivery model for cardiology patients in need of urgent care services during the weekend. This model creates a triage pathway for weekend cardiovascular ambulatory services to a day hospital instead of the emergency department.

There are an estimated five million active athletes at the high school, collegiate, professional and master's levels in the U.S. In a recent poll, cardiologists reported seeing five patient athletes per week on average. As the athletic population continues to expand in all major demographic groups, practices need a larger contingent of clinicians who understand the contemporary care and practice management strategies for athletes. Join this session to hear how a sports cardiology program can support and enrich the cardiovascular team.
