A Case Study: How Guideline-Directed Medical Therapy Decreased CHF Mortality

During this session, speakers will share a new strategic process improvement model deployed at UnityPoint Health. By utilizing a process improvement tool to collect clinician feedback, the multidisciplinary clinical effectiveness team identified priorities and improved care delivery for patients with chronic heart failure (CHF). Attendees will understand how a focus on guideline-directed medical therapy, standardization of care and performance tracking can improve medication adherence and decrease mortality rates in a complex patient population.
