Continuity Across the Lifespan: The Transition From Pediatric to ACHD Care Published on: May 2, 2026
Despite major advances in survival for patients with congenital heart disease (CHD), gaps in continuity of care, particularly during the transition from pediatric to adult congenital heart disease (ACHD) services, remain a significant challenge, with many adolescents lost to follow-up and limited workforce capacity to meet growing needs. This session will examine the current state of CHD care in the U.S. across the age spectrum, including trends in loss to follow-up, and review existing ACHD workforce gaps. Speakers will also highlight best practices for building successful transition programs that promote continuity and lifelong cardiovascular health, while addressing common challenges and barriers to implementing and sustaining these programs.
