Emergency medicine and cardiology have an opportunity to improve continuity of care for patients discharged after evaluation for suspected cardiac conditions, such as syncope and palpitations. In this webinar, Andrew Moore, MD, MCR, FACEP, reviewed current guidelines for emergency department (ED) triage and risk stratification, and the role of ambulatory cardiac monitoring in supporting care delivery.
Drawing on operational experience and early program learnings, the session outlined practical considerations for connected care pathways including patient selection, enrollment, cross-team communication and alert escalation. Attendees gained insights into strengthening care coordination and improving patient management across settings.
